This is a fully remote position, open to applicants in Washington.

📋 Description

• Deliver thorough medication management, which includes reconciliation, dose optimization, adherence support, and drug-level evaluations in line with evidence-based practices.

• Collaborate with physicians, specialists, and interdisciplinary teams to create personalized care plans under a Collaborative Practice Agreement (CPA) and facilitate smooth medication coordination.

• Assess the effectiveness and safety of therapies, pinpoint opportunities for optimization, and take appropriate action when necessary.

• Contribute to quality improvement and REMS initiatives.

• Assist colleagues with complex prior authorizations, enhance workflows, educate patients, and continue professional clinical development.

• Establish strong relationships with patients and providers through virtual assessments and monitoring of evidence-based treatment plans.


⛳️ Requirements

• Possess a Doctor of Pharmacy (PharmD) degree.

• Must hold a pharmacist license in Washington.

• Completion of a PGY2 Ambulatory Care Residency or a PGY1 Residency along with a minimum of 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA), or a minimum of 3 years of ambulatory practice with disease state management experience.

• Excellent communication, organizational, and technical skills.

• Demonstrate empathy, integrity, and a collaborative team-oriented approach.

• Strong computer skills are essential, including proficiency in Microsoft Office applications (Outlook, Teams, Word, Excel, and PowerPoint).

• Capable of navigating the internet and web-based platforms.

• Data entry and computer competencies will be assessed through a pre-employment evaluation.
