Clinical Pharmacist

This is a fully remote position, open to applicants in Illinois, +1 more state.

📋 Description

• Engage in remote work to assess, confirm, and provide guidance on Meds to Go prescriptions within Illinois.

• Support hospital communications and educational initiatives.

• Offer verification and consultation services for Medifriend locations and pharmacies in Wisconsin RD.

• Manage pharmaceutical care by sourcing and evaluating biomedical literature while creating medication therapy regimens.

• Advise patients and caregivers on medication therapy, potential side effects, adherence, appropriate usage, handling, and administration.

• Design, suggest, oversee, and evaluate individualized pharmacotherapeutic regimens for patients.

• Analyze and accurately interpret medication orders and prescriptions.

• Contribute to medical emergencies when necessary.

• Promote medication safety and engage in quality improvement activities, including reporting incidents.

• Conduct quality assurance and performance improvement for the medication-use system.

• Collaborate with the healthcare team regarding formulary compliance and the medication-use process.

• Assist with non-formulary, compassionate-use, and investigational medications.

• Prepare, dispense, and ensure proper storage and handling of medications.

• Act as a preceptor for pharmacy students, residents, and healthcare learners.

• Mentor, orient, train, and assist in performance evaluations for pharmacy colleagues, residents, and students.

• Supervise and verify the pharmacy support team’s work to ensure safe, efficient, and compliant workflows.

• Deliver age-appropriate patient care informed by growth, development, and patient-status data.

⛳️ Requirements

• Must hold a license in the respective state(s) of practice or be eligible for reciprocity.

• Certification in immunization and non-vaccine injectable administration necessary based on the practice area.

• Credentialing and privileging required based on the area of practice.

• A Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited institution is mandatory, or an FPGEC certificate from NABP.

• No prior experience required.

• Maintains proficiency in the area of practice.

• Proficient computer skills are essential.

• Exceptional communication and organizational abilities are required.

• Strong skills in presentation, facilitation, and training development.

• Decision-making, team-building, customer service, and conflict resolution skills are necessary.

• Life support training courses may be required based on departmental discretion.

• Must be licensed in both Illinois and Wisconsin.

• Availability to work first and second shifts, including weekends.

• Must meet physical demands, including walking, standing, pushing up to 50 pounds, and lifting 35 pounds.

• Proficiency in using pharmacy, computer, and automated dispensing equipment is required.

• Functional capabilities in speech, vision, and hearing are essential.
