Clinical Pharmacist, TX Licensed

This is a fully remote position, open to applicants in Texas.

📋 Description

• Deliver comprehensive, hands-on assistance to both patients and healthcare providers to enhance patient care and medication compliance.

• Assess clinician notes and evaluations for prior authorization approvals and recommendations regarding care plans.

• Create and enhance clinical protocols tailored to specific diseases and medications.

• Track patient compliance through PDC reports, evaluations, and mobile platform data, particularly for oncology and specialty conditions.

• Analyze EMR data, including drug-disease interactions, medication choices, lab results, vital signs, allergies, prescriptions, renewals, and communication with physicians.

• Suggest alternative therapeutic options and disease-specific treatments according to established guidelines.

• Supervise therapies for adverse effects and medication-related complications; advise prescribers on necessary modifications.

• Conduct medication therapy management, document interventions, and evaluate adherence rates.

• Execute pharmacist consultations, motivational interviewing, coaching, medication reconciliation, regimen optimization, and adherence interventions.

• Arrange home nurse visits and manage transitions between liaison/PSC teams.

• Track productivity and provide weekly growth reports.

• Oversee technicians’ daily responsibilities and confirm delivery coordination, prior authorizations, logs, and management of care utilization.

• Identify and address insurance challenges and financial assistance opportunities.

• Supervise communications with support personnel in accordance with HIPAA regulations.

• Liaise with healthcare professionals regarding patient care matters.

• Instruct pharmacy staff, students, interns, externs, and other healthcare providers.

• Manage and refresh data through database applications.

• Contribute to operational enhancements, productivity, quality, and customer service standards.

• Assist with research initiatives and peer-reviewed articles.

• Recognize trends, troubleshoot issues, propose enhancements, and facilitate change.

• Engage in process improvement efforts.

• Uphold confidentiality of sensitive information.

• Perform additional tasks as assigned.

⛳️ Requirements

• Candidates must possess a Doctor of Pharmacy (PharmD) or an equivalent advanced degree.

• A valid Pharmacist license is required.

• A minimum of 1 year of experience is highly preferred but not mandatory.

• Any additional licensure must be secured within a mutually agreed timeframe if necessary; continued employment depends on fulfilling these requirements.

• Familiarity with specialty medication pharmacology is essential.

• Understanding of local, state, and federal pharmacy regulations along with DEA requirements is necessary.

• Strong technical and clinical competencies are required.

• Proficiency in Microsoft Word, Excel, PowerPoint, Outlook, Teams, and other internet and web-based platforms is required.

• Knowledge of database and reporting tools is necessary.

• Familiarity with EPIC/QS1/WILLOW software is an advantage.

• Ability to identify obstacles to medication adherence and to develop and monitor improvement strategies.

• Capability to motivate and coach patients towards improved outcomes and to set therapy goals.

• Knowledge of complex/chronic disease states and specialty pharmacy therapies is preferred.

• Excellent verbal and written communication abilities are essential.

• Proficiency in managing processes and personnel is required.

• Computer and data entry skills will be evaluated during the pre-employment process.

• Must retain registration and adhere to local, state, and federal drug laws and relevant state board of pharmacy and DEA regulations.
