Clinical Pharmacist – HEDIS Medication Adherence

This is a fully remote position, open to applicants in United States.

📋 Description

• Spearhead initiatives aimed at enhancing HEDIS medication adherence and pharmacy quality metrics.

• Analyze data on medication adherence, pinpoint care gaps, and execute targeted interventions.

• Work collaboratively with healthcare providers, care managers, and stakeholders to develop adherence strategies.

• Facilitate medication therapy management (MTM) sessions and deliver patient education.

• Identify and address barriers to medication adherence, including financial constraints, side effects, and complex dosing schedules.

• Employ motivational interviewing techniques to promote adherence.

• Monitor and evaluate HEDIS performance metrics while producing effectiveness reports.

• Utilize EHR and pharmacy data to pinpoint patients at risk for non-adherence and formulate action plans.

• Provide leadership with updates on progress and recommendations for improvement.

• Act as the organization’s expert in medication adherence.

• Train and support pharmacists, care managers, and clinical staff on best practices for medication adherence.

• Engage in cross-functional initiatives related to quality improvement, population health management, and value-based care.

• Adhere to security policies and procedures to safeguard patient and company data.

• Remain accessible during designated working hours and engage in virtual meetings with cameras activated.

⛳️ Requirements

• Doctor of Pharmacy (PharmD) degree from an accredited pharmacy school.

• Active pharmacist license; willingness to secure licenses in TX, TN, AL, and PA, funded by the company.

• At least 2 years of experience in HEDIS-related medication adherence, quality improvement, or population health.

• In-depth understanding of HEDIS measures, specifically those related to medication adherence.

• Proficient in electronic health record (EHR) systems and data analysis tools.

• Exceptional communication, problem-solving, and organizational abilities.

• Capability to work independently as well as within a team-oriented setting.

• Experience in motivational interviewing or similar patient engagement techniques is advantageous.

• Background in managed care, pharmacy benefits management (PBM), or healthcare quality improvement is preferred.

• Certification in Medication Therapy Management (MTM), Certified Geriatric Pharmacist (CGP), or other relevant qualifications.

• Comprehension of value-based care models and their influence on medication adherence.

• Availability during specified working hours and participation in virtual meetings with cameras on.

• Internet connection capable of supporting video calls and access to internal/external systems, with minimum speeds of 100 Mbps download and 10 Mbps upload.

• Accountability for adhering to security policies and procedures to protect PHI, PII, and company intellectual property.
