This is a fully remote position, open to applicants in Texas.
• Direct initiatives aimed at enhancing HEDIS medication adherence and pharmacy quality metrics.
• Evaluate medication adherence data, pinpoint care gaps, and execute targeted interventions.
• Partner with providers, care managers, and stakeholders to formulate adherence strategies.
• Facilitate medication therapy management sessions and offer patient education.
• Recognize and address adherence obstacles such as financial issues, side effects, and intricate dosing regimens.
• Employ motivational interviewing techniques to promote adherence.
• Track HEDIS performance indicators and prepare effectiveness reports.
• Utilize EHR and pharmacy data to identify patients at risk and create action plans.
• Update leadership on progress towards HEDIS objectives and suggest enhancements.
• Act as the organization's expert in medication adherence.
• Educate and assist pharmacists, care managers, and clinical personnel.
• Engage in quality improvement, population health management, and value-based care initiatives.
• Adhere to security protocols to safeguard PHI, PII, and company intellectual property.
• Doctor of Pharmacy (PharmD) degree from an accredited pharmacy institution.
• Valid pharmacist license; must be willing to obtain licenses in TX, TN, AL, and PA (company funded).
• At least 2 years of experience in HEDIS-related medication adherence, quality improvement, or population health.
• In-depth understanding of HEDIS measures, especially those pertaining to medication adherence.
• Competence in EHR systems and data analysis tools.
• Familiarity with motivational interviewing or similar patient engagement methods is advantageous.
• Background in a managed care environment, pharmacy benefits management (PBM), or healthcare quality improvement is preferred.
• Certification in Medication Therapy Management (MTM), Certified Geriatric Pharmacist (CGP), or other relevant qualifications is preferred.
• Knowledge of value-based care models and their influence on medication adherence is preferred.
• Personal internet connection with at least 100 Mbps download and 10 Mbps upload speed.
