This is a fully remote position, open to applicants in Texas.
• Lead initiatives aimed at enhancing HEDIS medication adherence and pharmacy quality metrics.
• Examine medication adherence data, pinpoint care deficiencies, and execute targeted interventions.
• Partner with healthcare providers, care managers, and stakeholders to develop adherence strategies.
• Conduct sessions for medication therapy management (MTM).
• Educate patients and address barriers related to cost, side effects, and dosing.
• Utilize motivational interviewing techniques to promote medication adherence.
• Oversee and monitor HEDIS performance indicators.
• Create reports evaluating the effectiveness of interventions and programs.
• Leverage EHR and pharmacy data to identify patients at risk of non-adherence and formulate action plans.
• Provide leadership with updates on progress and recommendations for improvement.
• Act as the organization's expert on medication adherence.
• Train and support pharmacists, care managers, and clinical staff.
• Engage in cross-disciplinary quality improvement, population health management, and value-based care initiatives.
• Adhere to security policies and procedures to safeguard patient and company information.
• Doctor of Pharmacy (PharmD) degree from a recognized school of pharmacy.
• Current pharmacist license; willingness to obtain licenses in TX, TN, AL, and PA (company sponsored).
• At least 2 years of experience in HEDIS-related medication adherence, quality improvement, or population health.
• In-depth knowledge of HEDIS measures, especially those related to medication adherence.
• Proficient in electronic health record (EHR) systems and data analysis tools.
• Strong communication, problem-solving, and organizational abilities.
• Capability to work independently as well as collaboratively in a team environment.
• Experience with motivational interviewing or similar patient engagement methods is advantageous.
• Background in managed care, pharmacy benefits management (PBM), or healthcare quality improvement is beneficial.
• Certification in Medication Therapy Management (MTM), Certified Geriatric Pharmacist (CGP), or other pertinent credentials.
• Familiarity with value-based care models and their influence on medication adherence.

• Employees must be reachable during designated working hours.
• Willingness to participate in virtual meetings with cameras enabled.
