Clinical Pharmacist – HEDIS Medication Adherence

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Spearhead initiatives to enhance HEDIS medication adherence and pharmacy quality metrics.

• Evaluate medication adherence data, pinpoint care gaps, and execute targeted interventions.

• Partner with providers, care managers, and stakeholders to devise adherence strategies.

• Facilitate medication therapy management sessions and offer patient education.

• Recognize and mitigate adherence obstacles such as cost, side effects, and intricate dosing schedules.

• Employ motivational interviewing techniques to promote adherence.

• Track HEDIS performance indicators and produce effectiveness reports.

• Utilize EHR and pharmacy data to identify patients at risk of non-adherence and formulate action plans.

• Provide leadership with updates on progress and recommendations for improvements.

• Act as the subject matter expert on medication adherence.

• Train and assist pharmacists, care managers, and clinical personnel.

• Engage in quality improvement, population health, and value-based care projects.

• Adhere to security protocols to safeguard PHI, PII, and company intellectual property.

• Disclose any secondary employment and ensure availability during designated working hours.

⛳️ Requirements

• Doctor of Pharmacy (PharmD) degree from an accredited pharmacy school.

• Active pharmacist license; willing to obtain licenses in TX, TN, AL, and PA (cost covered by the company).

• At least 2 years of experience in HEDIS-related medication adherence, quality improvement, or population health.

• In-depth knowledge of HEDIS measures, especially those related to medication adherence.

• Proficient in EHR systems and data analysis tools.

• Exceptional communication, problem-solving, and organizational abilities.

• Capacity to work independently as well as collaboratively within a team environment.

• Experience with motivational interviewing or similar patient engagement strategies is advantageous.

• Background in managed care, PBM, or healthcare quality improvement is preferred.

• Certification in MTM, CGP, or other relevant credentials is preferred.

• Familiarity with value-based care models and their influence on medication adherence is preferred.

• Ability to work during designated hours and participate in virtual meetings with cameras on.

• Minimum internet speed of 100 Mbps download and 10 Mbps upload is required.
