Clinical Pharmacist – HEDIS Medication Adherence

This is a fully remote position, open to applicants in Alabama.

📋 Description

• Spearhead initiatives aimed at enhancing HEDIS medication adherence and pharmacy quality metrics.

• Evaluate medication adherence data, pinpoint care gaps, and implement focused interventions.

• Partner with healthcare providers, care managers, and stakeholders to develop adherence strategies.

• Conduct medication therapy management sessions while providing education to patients.

• Identify and address barriers to adherence, such as costs, side effects, and complex dosing schedules.

• Utilize motivational interviewing techniques to promote adherence.

• Track HEDIS performance metrics and create effectiveness reports.

• Leverage EHR and pharmacy data to identify patients at risk for non-adherence and formulate action plans.

• Update leadership on progress and provide recommendations for improvement.

• Act as the subject matter expert on medication adherence.

• Train and assist pharmacists, care managers, and clinical staff.

• Engage in quality improvement, population health management, and value-based care teams.

• Adhere to security policies that protect PHI, PII, and the company's intellectual property.

• Disclose any secondary employment.

⛳️ Requirements

• Doctor of Pharmacy (PharmD) degree from an accredited pharmacy school.

• Active pharmacist license; willing to obtain licenses in TX, TN, AL, and PA (company-sponsored).

• At least 2 years of experience in HEDIS-related medication adherence, quality improvement, or population health.

• Strong understanding of HEDIS measures, especially medication adherence metrics.

• Proficient in EHR systems and data analysis tools.

• Exceptional communication, problem-solving, and organizational abilities.

• Capacity to work autonomously as well as collaboratively in a team-oriented environment.

• Experience with motivational interviewing or similar patient engagement methods is advantageous.

• Background in a managed care, PBM, or healthcare quality improvement setting is preferred.

• Certification in Medication Therapy Management (MTM), Certified Geriatric Pharmacist (CGP), or other relevant credentials is a plus.

• Knowledge of value-based care models and their implications for medication adherence.

• Employees should be reachable during designated working hours.

• Team members are expected to participate in virtual meetings with cameras on.
