This is a fully remote position, open to applicants in Alabama, +29 more states.
• Engage directly with patients to oversee chronic conditions through customized care plans.
• Create and deliver educational resources specifically designed for the local community's needs.
• Work in collaboration with interdisciplinary teams to enhance disease management strategies.
• Assist members and providers in navigating benefits to guarantee accessible and affordable healthcare.
• Conduct thorough medication assessments for members with intricate medication regimens.
• Detect and resolve drug therapy challenges to enhance care and improve patient outcomes.
• Propel “5-Star” performance on clinical measures related to pharmacy.
• Tackle therapy gaps, such as initiating statin therapy for diabetes or cardiovascular wellness.
• Interact empathetically with members to aid them in achieving personal health objectives.
• Provide support during on-call rotations for urgent pharmacy requirements.
• Serve as a clinical expert in the development of pharmacy models.
• Mentor and educate the broader team on pharmacotherapy and practice-related subjects.
• Employ AI-driven clinical tools to assist with documentation and the identification of drug therapy issues.
• Offer feedback to enhance tool accuracy and improve workflow efficiency.
• Obtain additional state licensure promptly.
• Execute other responsibilities as assigned.
• Report to the Senior Manager, Clinical Pharmacy, with indirect reporting lines to other clinical and operational leaders.
• A minimum of 3 years of clinical experience in direct patient care.
• Active state licensure in good standing required in at least one of the following states: Alabama, Florida, Georgia, Indiana, Missouri, Ohio, North Carolina, South Carolina, Texas.
• Must obtain licensure in at least 5 states, with additional states as necessary.
• Licensure must be in good standing, free from restrictions or sanctions.
• Travel may be necessary to obtain additional licensure.
• Reside within the Central, Mountain, Pacific, or Hawaiian time zones, or be able to support work during typical clinic hours in these zones.
• Fluent proficiency in Spanish is required.
• Experience with the implementation or operation under collaborative pharmacy practice agreements is preferred.
• Board certification or experience in complex geriatric care, behavioral health, and/or lipid management is preferred.
• Comfortable using AI-assisted clinical tools and providing feedback for improvement.
• Licensure in 3 or more specified states at the time of application is desired.
• Additional language proficiency will be considered favorably.
