Clinical Pharmacist – Clinical Pharmacy Programs

📋 Description

• Engage directly with patients to manage chronic conditions through customized care plans.

• Create and deliver educational resources tailored to the unique needs of the local community.

• Collaborate with multidisciplinary teams to refine disease management procedures.

• Assist members and providers in navigating benefits to ensure healthcare accessibility and affordability.

• Perform thorough medication reviews for members with intricate medication regimens.

• Identify and address drug therapy challenges to enhance care and improve health outcomes.

• Strive for “5-Star” performance in pharmacy-related clinical metrics.

• Tackle therapy gaps, such as statin initiation for diabetes or cardiovascular health.

• Interact empathetically with members to support them in achieving their personal health objectives.

• Provide support during on-call rotations for urgent pharmacy needs.

• Serve as a clinical expert in pharmacy model development.

• Mentor and educate the broader team on pharmacotherapy and practice-related subjects.

• Leverage AI-enabled clinical tools for documentation and drug therapy issue identification, providing feedback to enhance accuracy and workflow efficiency.

• Obtain additional state licensure promptly.

• Perform other assigned responsibilities.

• Report to the Senior Manager, Clinical Pharmacy, with indirect reporting relationships to other clinical and operational leadership.


⛳️ Requirements

• A minimum of 3 years of clinical experience in direct patient care is required.

• Current state licensure in good standing is mandatory in Alabama, Florida, Georgia, Hawai’i, Indiana, Missouri, Ohio, North Carolina, South Carolina, or Texas.

• Must obtain licensure in at least 5 states, with additional states as necessary.

• Licenses must be unrestricted and free from sanctions.

• Travel may be necessary to obtain additional licensure.

• Experience with the implementation or operation under collaborative pharmacy practice agreements is preferred.

• Board certification or specific experience in complex geriatric care, behavioral health, and/or lipid management is preferred.

• Comfort in utilizing AI-assisted clinical tools and providing constructive feedback for their improvement is essential.

• Licensure in 3 or more specified states at the time of application is desired.
