This is a fully remote position, open to applicants in Puerto Rico.
• Engage in patient outreach and handle incoming calls to deliver pharmacist services.
• Provide Medication Therapy Management services, encompassing Comprehensive Medication Reviews and Targeted Medication Reviews.
• Execute medication reconciliation, prior authorization assessments, and various clinical services.
• Conduct personalized asynchronous video consultations and follow-ups.
• Evaluate medication therapy for unnecessary medications, additional medication needs, inappropriate dosages, adverse drug reactions, ineffective medications, and adherence challenges.
• Record clinical recommendations, therapy responses, and notes on patient care.
• Forward clinical recommendations to provider offices and monitor implementation.
• Collaborate with nurses, health coaches, management, product managers, and client-facing personnel.
• Achieve productivity and performance targets.
• Create and refine operational policies and procedures for pharmacy services.
• Discover innovative methods to enhance processes and workflows.
• Advise Product and Experience teams on potential product improvements.
• Recruit, train, mentor, counsel, and assess direct reports as needed.
• Must be a resident of Puerto Rico and maintain residency throughout employment.
• Must have legal authorization to work in Puerto Rico/United States without employer sponsorship.
• Active Pharmacist License is required.
• Minimum of 3 years of experience in pharmacy practice.
• Fluency in both English and Spanish, including reading, writing, speaking, and comprehension.
• Capability to obtain licensure in additional states as necessary.
• Strong telephonic communication skills, particularly when serving elderly patients.
• Excellent communication abilities and capacity to build rapport with patients.
• Outstanding listening and interpersonal skills.
• Ability to work independently and as part of a virtual/decentralized team.
• Competence in safeguarding confidential information.
• Self-motivated with the capacity to multitask effectively.
• Proficient in Microsoft Teams, Outlook, Word, Excel, and other software programs.
• Must be able to participate in meetings and engage with patients via video.
• Preferred: experience in digital health programs or startups in the health sector.
• Preferred: background with hospitals or health plans focused on quality improvement.
• Preferred: familiarity with Salesforce and clinical documentation platforms.
• Sales or call center experience is preferred but not mandatory.
