This is a fully remote position, open to applicants in United States.
• Operate within clinical programs led by pharmacists that focus on chronic illness, quality enhancement, and medication optimization.
• Engage, enroll, and outreach to eligible patients through education, follow-up, and telephonic medication support.
• Conduct Medication Reconciliation, Medication Therapy Management, and Comprehensive Medication Reviews.
• Provide education on medication use, adherence, self-management, and escalation of care.
• Make actionable recommendations regarding medications and collaborate with providers to aid in medication management and practice adoption.
• Support clinical programs by offering pharmacy-related consultation.
• Promote medication adherence and address quality gaps in care.
• Work with interdisciplinary teams to create patient-specific clinical action plans, reconcile medications, and make necessary adjustments to medications.
• Encourage provider engagement with drug-related clinical decision-support recommendations.
• Offer feedback for improving decision-support logic, usability, and overall impact.
• Collaborate with medical officers, directors, practice transformation managers, care teams, and various practices.
• Ensure consistent clinical standards, workflows, and messaging across different markets.
• Assist in the development of specialty medication, high-cost drug, and pharmacy optimization initiatives.
• Partner with pharmacy and medical leadership to implement a value-based drug strategy.
• Document clinical activities in accordance with state and federal regulations and HIPAA requirements.
• Serve as a pharmacy information resource for both providers and internal clinical teams.
• Stay updated on clinical guidelines, quality measures, and pharmacy best practices.
• Doctor of Pharmacy (PharmD) degree.
• Active pharmacist license in good standing in one of the following states: CA, WA, KS, OH, NY, NJ, FL, MD, NH, MS, LA.
• Experience in Medication Therapy Management and pharmacist-led clinical programs.
• Strong comprehension of evidence-based guidelines and quality measures.
• Experience or interest in value-based care, population health, or ACO settings.
• Demonstrated clinical judgment with the capability to make independent recommendations.
• Ability to collaborate across clinical programs, data-driven tools, and operational teams.
• Comfort in handling partial or evolving clinical information.
• Results-oriented mindset, focused on driving outcomes rather than merely providing clinical input.
• Excellent communication skills when interacting with providers, patients, and internal stakeholders.
• Organized and adaptable, thriving in fast-paced, changing environments.
• High level of professionalism, integrity, and accountability.
• Eligibility to work in the United States; application includes a question regarding sponsorship needs.
