This is a fully remote position, open to applicants in United States.
• Enhance medication management to allow physicians to allocate more time to patient care and facilitate care coordination for designated patient cohorts.
• Collaborate closely with the care coordination team and physicians to guarantee that patients receive the intended therapies, comprehend their medication regimens, and benefit from adherence coaching.
• Conduct telephonic follow-ups for medication reconciliation and management, which includes transitional care and support for chronic diseases.
• Complete designated medication management tasks and appointments following prescribed workflows.
• Oversee transitional management tasks, such as obtaining medication orders, performing medication reconciliation, and ensuring medication availability until primary care physician follow-up.
• Evaluate patient adherence behaviors, identify barriers to adherence, and assess the appropriateness of therapy.
• Document services within the Engage application, EnterpriseRx, and care management systems.
• Collaborate with central or local pharmacies to obtain and process prescriptions.
• Communicate with providers regarding the services rendered.
• Aid in achieving member education objectives related to specific chronic conditions as outlined in the Plan of Care.
• Contribute to the advancement of pharmacy service delivery for key patient populations, if applicable.
• Execute tasks specified in member-specific Plans of Care.
• Adhere to customer service expectations, departmental policies, procedures, and job scope limitations.
• Engage in departmental and company in-service training sessions.
• Perform additional related duties as assigned.
• Doctor of Pharmacy (PharmD).
• Valid Texas state pharmacy license and/or license for the assigned market.
• Good standing residential state license.
• Clinically required licenses maintained in good standing.
• Licenses required for Pharmacist-in-Charge (PIC) in good standing.
• Strong skills in patient engagement, including active listening and de-escalation intervention training.
• Capability to explain complex topics in a clear and understandable manner.
• Excellent organizational and time management abilities.
• Proficiency in working independently while managing multiple projects simultaneously.
• Exceptional written and verbal communication skills, particularly in professional telephone interactions.
• Proficient in Microsoft Word and Excel.
