Clinical Pharmacist

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Perform thorough and focused Medication Safety Reviews for PACE participants utilizing specialized clinical platforms.

• Create personalized, quantifiable, and goal-driven therapeutic care plans.

• Execute medication reviews and reconciliations during new enrollments, annual assessments, significant changes, care transitions, and other high-risk clinical situations.

• Detect, assess, document, and promote the resolution of medication-related issues, including polypharmacy, adverse drug events, unsuitable medications, therapeutic deficiencies, adherence obstacles, and medication access issues.

• Collaborate with providers, interdisciplinary team members, pharmacists, and healthcare professionals to provide clinical recommendations and enhance medication optimization.

• Engage providers through written medication safety evaluations, clinical consultations, polypharmacy assessments, fall risk analyses, and care transition reviews.

• Track and document outcomes and progress towards therapeutic objectives.

• Supervise participants on high-risk medications or managing intricate chronic disease regimens.

• Take part in client meetings, interdisciplinary discussions, Pharmacy and Therapeutics Committee meetings, drug utilization assessments, quality enhancement initiatives, and educational programs.

• Ensure compliance with federal and state regulations, accreditation standards, and organizational policies pertaining to pharmacy practice, including quality assurance, HIPAA, Fraud, Waste and Abuse, and patient safety reporting.

⛳️ Requirements

• Graduate from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE) or a qualifying program outside the U.S.

• Current, active license/registration in good standing with the state board of pharmacy where the practice will occur.

• Preferred experience in direct interaction with prescribers and patients.

• Preferred experience or strong interest in clinical pharmacogenomics.

• Above-average knowledge of geriatric pharmacotherapy and the ability to apply it in clinical settings.

• Capability to identify and resolve complex medication-related issues in geriatric patients with concurrent illnesses and multiple medications.

• Strong analytical abilities, clinical judgment, and decision-making skills.

• Self-motivated, committed to professional development, accountable, and capable of following through.

• Effective professional communication skills, both written and verbal, as well as strong interpersonal skills.

• Proficient technical and computer skills, with the ability to navigate proprietary and pharmacy software.

• Proven problem-solving, conflict-resolution, and project-management abilities.

• Detail-oriented and organized; able to perform under pressure, multitask, meet deadlines, and uphold quality standards.

• Knowledge of state and federal laws and regulations, including HIPAA and Fraud, Waste, and Abuse.

• Active BPS certification in geriatrics (BCGP) is preferred.

• Availability to work an average of 40 hours per week and occasionally outside normal business hours.
