Case Manager, Registered Nurse

This is a fully remote position, open to applicants in Texas.

📋 Description

• Perform telephonic and occasional face-to-face assessments to gauge member requirements.

• Create and oversee customized care plans aimed at enhancing health and wellness.

• Facilitate care coordination across various programs, providers, and resources.

• Utilize clinical judgment to identify risks and eliminate barriers to care.

• Engage members in impactful conversations to assist them in achieving their health objectives.

• Work collaboratively with a multidisciplinary team to maximize outcomes.

⛳️ Requirements

• Active, unrestricted multistate RN license in your state of residence.

• A minimum of 3 years of clinical nursing experience.

• Proficient computer skills, including Microsoft Office and web-based systems.

• Capability to adhere to the required schedule.

• An Associate's Degree in Nursing is mandatory.

• Experience in case management (integrated model) is preferred.

• Bilingual proficiency in Spanish and English is preferred.

• A Bachelor's degree in Science & Nursing is preferred.
