Case Manager, Registered Nurse

This is a fully remote position, open to applicants in Arizona.

📋 Description

• Evaluate, strategize, implement, and coordinate member care through telephonic and/or virtual means.

• Create personalized care plans and wellness initiatives.

• Recognize clinical and social obstacles affecting health outcomes.

• Connect members with available programs, benefits, and community resources.

• Utilize clinical judgment to manage intricate health conditions and mitigate risk factors.

• Engage with members using effective communication and motivational interviewing techniques.

• Operate in a queue-style environment.

• Work Monday through Friday, from 9:00 AM to 5:30 PM EST.

• Participate in occasional on-call weekends and holidays on a voluntary basis.

• Collaborate with members, providers, and care teams to enhance outcomes.

⛳️ Requirements

• Active, unrestricted RN license in your state of residence.

• Willingness to acquire additional state licenses.

• Over 3 years of acute care RN experience.

• Associate Degree in nursing.

• Proficient in using multiple computer applications and monitors.

• Compact RN license is preferred.

• BSN is preferred.

• At least 1 year of case management experience is preferred.

• Certification as a Certified Case Manager (CCM) is preferred.

• Strong customer service and member engagement abilities.

• Excellent communication skills via phone.

• Direct/hardwired internet connection to a modem/router within 7 feet of the computer is required.

• Minimum internet speed of 25 Mbps download and 3 Mbps upload is necessary.

• WiFi and satellite internet connections are not allowed.

• A quiet, secure, and private designated home workspace that complies with CVS Health and HIPAA guidelines.

• Must be able to work within the state and city of current residence.

• Experience with Microsoft Office 365 or similar Google Workspace applications.

• Capability to use VPN and EHR portals.

• Ability to manage strong passwords and identify suspicious emails or links.

• Skills to troubleshoot common technical issues.

• Willingness to learn new skills.

• Less than 5% travel for meetings, training, or licensure requirements.
