This is a fully remote position, open to applicants in Arizona, +15 more states.

📋 Description

• Facilitate care coordination across inpatient, outpatient, emergency room, and home care environments.

• Collaborate with members and providers to navigate complex health issues and develop personalized care strategies.

• Evaluate and address Social Determinants of Health by linking members to customized community support services.

• Offer crisis intervention assistance and advocate for members to eliminate barriers to care and ensure treatment adherence.

• Engage with members via phone, secure messaging, or face-to-face to provide education on health conditions, hospital discharges, medical procedures, and new prescriptions.

• Uphold strict confidentiality and maintain high professional standards.

• Ensure adherence to relevant laws and regulations.

• Participate in process improvement initiatives and new pilot programs.

• Carry out additional responsibilities as assigned.

• Report to the Manager of Clinical Programs.

⛳️ Requirements

• Minimum of 2 years of clinical experience in hospital, outpatient, or community-based care management.

• At least 2 years of experience working directly with patients managing chronic medical conditions, such as diabetes, COPD, or hypertension.

• Over 1 year of experience in Care Coordination and Navigation.

• Possession of an active, unrestricted RN license from the United States in the relevant state, or an active compact multistate unrestricted RN license, or a Master of Social Work (MSW) from a Nationally Accredited School recognized by the Council on Social Work Education.

• BSN, Licensed Master Social Worker or Licensed Clinical Social Worker, CCM Certification, experience in Case Management, Complex Case Management certification, and proficiency in Spanish are advantageous.
