This is a fully remote position, open to applicants in New York.
• Evaluates clients' requirements and eligibility for in-home services, benefits, and entitlements, while overseeing ongoing casework and care plan management.
• Conducts intakes and provides precise information and referrals.
• Screens and aids clients with applications and re-certifications for benefits and entitlements.
• Advocates for clients and organizes service provision; develops, implements, and follows up on care plans.
• Offers supportive counseling to clients as necessary.
• Maintains continuous communication with clients, family members, and other relevant parties as appropriate.
• Ensures complete and accurate case notes by utilizing the program’s data system; prepares necessary documentation in the case file within organizational deadlines.
• Maintains and reports statistics in accordance with Selfhelp standards, using the program’s data system and other record-keeping methods as required.
• Actively engages in supervision and staff meetings, promptly informing the supervisor of any emerging issues.
• Facilitates and assists with social, recreational, and other group activities and events.
• Performs any additional duties as assigned.
• Bachelor's Degree is mandatory.
• Proficiency in Russian and English is required.
• A minimum of one year of related experience and/or training.
• Familiarity with Microsoft Office Suite and other technological tools.
