Bilingual Advanced Practice Nurse, Memory Support

This is a fully remote position, open to applicants in Florida.

📋 Description

• Perform thorough telehealth evaluations for patients experiencing memory issues, which include taking detailed clinical histories and making remote physical/neurological observations.

• Utilize and interpret validated remote assessment instruments (e.g., Mini-Cog, MoCA, SLUMS) to identify the type and stage of cognitive impairment.

• Diagnose and manage neuropsychiatric issues such as agitation, anxiety, depression, and sleep disturbances through both pharmacological and non-pharmacological methods.

• Provide education to families regarding disease progression, safety planning (including wandering and driving), and techniques for behavioral de-escalation.

• Ensure electronic health records (EHR) are maintained in accordance with Florida state regulations and HIPAA requirements for telehealth.

⛳️ Requirements

• Must possess a valid, unencumbered Florida APRN license that includes prescriptive authority.

• Hold a Master's or Doctoral degree in Nursing (MSN or DNP).

• Preferably have 2–3 years of experience in geriatrics, neurology, or a specialized memory care setting.

• Experience in conducting video-based clinical consultations and the ability to troubleshoot basic technology issues with elderly patients and their families.

• Familiarity with the CMS Cognitive Assessment & Care Plan requirements and the specific aspects of dementia-related behavioral health.
