Case Management Analyst

This is a fully remote position, open to applicants in New York.

📋 Description

• Deliver comprehensive care management services to participants enrolled in the New York Managed Long Term Care (MLTC) Plan.

• Perform psychosocial assessments to determine member needs.

• Create and execute individualized service plans centered on the member.

• Coordinate long-term services and support options.

• Assist members in accessing medical, behavioral health, and community resources.

• Collaborate effectively with members, caregivers, providers, and interdisciplinary teams.

• Encourage member independence and enhance health outcomes.

• Address social determinants affecting health.

• Ensure the provision of quality, cost-effective care.

• Conduct member visits within the designated service area as required.

⛳️ Requirements

• Possession of an active and unrestricted New York State Social Work license (LMSW or LCSW) as necessary.

• A minimum of two years of experience in care management, case management, managed care, behavioral health, long-term care, home care, or a related healthcare environment.

• Familiarity with New York Medicaid, Medicare, Managed Long Term Care (MLTC), and Long-Term Services and Supports (LTSS).

• Experience in conducting psychosocial assessments and formulating person-centered care plans.

• Excellent communication, care coordination, advocacy, organizational, and problem-solving abilities.

• Capability to work in collaboration with interdisciplinary teams, providers, members, and caregivers.

• Proficiency in Microsoft Office applications and electronic documentation systems.

• Willingness to travel within the assigned service area to carry out member visits as necessary.

• A Master's Degree in Social Work (MSW) from an accredited institution is mandatory.

• Active New York State LMSW or LCSW licensure is preferred or required depending on business requirements.

• Equivalent combinations of education and relevant care management experience may be assessed where applicable.
