This is a fully remote position, open to applicants in Michigan.

📋 Description

• Establish trusting relationships with patients through motivational communication techniques.

• Assess vital signs and utilize data to encourage positive lifestyle and health behavior modifications.

• Work with patients’ care teams to set patient-focused goals and action plans.

• Actively monitor electronically transmitted blood pressure, weight, and blood glucose data.

• Conduct monthly wellness calls with designated patients.

• Facilitate collaborative wellness calls to outline health objectives.

• Oversee patient messaging and alerts.

• Refer patients to treating physicians for routine inquiries.

• Deliver preventive health and disease management education and coaching.

• Adhere to escalation protocols for urgent care requirements.

• Engage in synchronous and asynchronous patient communication via Vivo Care’s Remote Care Platform.

• Achieve team and patient engagement targets and standards.

• Foster ongoing patient engagement in RPM programs.


⛳️ Requirements

• Valid RN/LPN/LVN license in a compact state and Michigan.

• Availability to work in the EST zone from 8:00 AM to 5:00 PM.

• 1-3 years of clinical experience, patient management, or disease management is preferred.

• Familiarity with various provider practices and workflows.

• Quick learning ability.

• Capability to work independently with little supervision.

• Experience with Medicare patients.

• Background in conducting virtual visits with patients and telephonic care management.

• Spanish language proficiency is a plus.

• Experience with Motivational Interviewing/Health Behavior Change is advantageous.

• Health Coach certification is a bonus.

• Clinical experience with adult and geriatric patient populations.

• Proficiency with remote working technologies.

• Understanding of remote-patient monitoring (RPM), Chronic Care Management (CCM), Behavioral Health, Care Coordination, or Utilization Management principles is preferred.

• Strong awareness of reimbursement requirements.
