This is a fully remote position, open to applicants in United States.
• Oversee physiological indicators such as blood pressure, weight, and blood glucose in a clinically appropriate manner.
• Achieve team objectives, standards, and engagement metrics.
• Foster ongoing patient involvement in remote patient monitoring initiatives.
• Deliver education and coaching on preventive health and disease management.
• Conduct monthly wellness calls with designated patients.
• Facilitate collaborative wellness calls to establish health goals with patients and their Care Teams.
• Manage patient messaging and alerts effectively.
• Guide patients to their treating physicians for routine inquiries.
• Adhere to escalation protocols for urgent care needs.
• Assess vital signs and utilize data to encourage positive lifestyle and health behavior changes.
• Create patient-focused goals and related action plans in partnership with Care Teams.
• Monitor patients and engage in synchronous and asynchronous communication via Vivo Care’s Remote Care Platform.
• Active RN/LPN/LVN license in a compact state without restrictions.
• Availability to work in the MST time zone from 8:00 AM to 5:00 PM.
• 1-3 years of clinical experience, including patient management or disease management, preferred.
• Familiarity with various provider practices and workflows.
• Capability to work independently with minimal supervision.
• Experience with Medicare patient populations.
• Background in conducting virtual visits and telephonic care management.
• Proficiency in Spanish is a plus.
• Experience in Motivational Interviewing/Health Behavior Change is a plus.
• Health Coach certification is an advantage.
• Clinical experience working with adult and geriatric patient populations.
• Ideally, experience in phone triage.
• Preferred knowledge of remote patient monitoring (RPM) and/or Chronic Care Management (CCM), Behavioral Health, Care Coordination, or Utilization Management principles.
• Familiarity with remote working technologies.
• Strong understanding of reimbursement requirements.
• Weekends, holidays, and evenings/nights off.
