Care Navigator – Foster Care

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Design, evaluate, and oversee care management activities tailored to member requirements.

• Create or assist in the development of individualized care/service plans.

• Provide education to members and their families/caregivers regarding services, benefits, care options, referrals, and provider guidelines.

• Assess member needs, obstacles to care, available resources, and suggested outcomes.

• Identify healthcare providers, specialists, and community resources essential for care.

• Offer psychosocial and resource assistance related to employment, education, housing, nutrition, participant direction, independent living, justice, and foster care.

• Facilitate coordination between members/families/caregivers and care provider teams to ensure prompt access to care and services.

• Track progress toward the goals established in care plans and monitor changes in member status or condition.

• Collaborate with healthcare professionals to update care plans and respond to member needs.

• Gather, document, and maintain member information and care management activities in compliance with regulations.

• Conduct on-site visits when necessary.

• Execute other duties as assigned while adhering to all policies and standards.

⛳️ Requirements

• Bachelor’s degree.

• 2–4 years of relevant experience.

• Graduation from an Accredited School of Nursing if possessing clinical licensure.

• Current state clinical license is preferred.

• Ability to work remotely from any location within the state of Missouri.

• Availability to work Monday through Friday, from 8am to 5pm.

• Previous experience in foster care is highly desirable.
