Care Navigator – Foster Care

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Create, evaluate, and manage care coordination activities tailored to member needs.

• Develop or assist in crafting individualized care plans or service plans.

• Provide education to members and their families or caregivers regarding services, benefits, procedures, care alternatives, referrals, and provider guidelines.

• Assess member needs, identify barriers to care, evaluate available resources, and recommend or facilitate suitable outcomes.

• Identify necessary providers, specialists, and community resources for care delivery.

• Offer psychosocial and resource assistance related to employment, education, housing, food, participant direction, independent living, justice, and foster care.

• Coordinate with members, families, and caregivers alongside care provider teams to ensure prompt access to services.

• Track progress toward care plan objectives and monitor changes in member status or condition.

• Collaborate with healthcare professionals on care plan modifications and refer members for additional care management evaluations.

• Gather, document, and maintain member information and care management activities to ensure regulatory compliance.

• Conduct on-site visits when necessary to assess needs and collaborate with providers or resources.

• Perform other assigned responsibilities while adhering to company policies and standards.

⛳️ Requirements

• A bachelor's degree with 2 to 4 years of relevant experience.

• Graduation from an Accredited School of Nursing if holding a clinical license.

• Equivalent experience demonstrating applicable knowledge, responsibilities, and skills reflective of the position level may be acceptable.

• Current clinical license from the state is preferred.

• Availability to work Monday through Friday, from 8 am to 5 pm.

• Previous experience in foster care is highly preferred.

• Candidates must be able to work remotely from their home anywhere within the state of Missouri.
