Care Navigator – Foster Care

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Design, evaluate, and manage care coordination activities tailored to the needs of members.

• Create or assist in the development of individualized care and service plans.

• Inform members and their families/caregivers about available services, benefits, and care options.

• Assess member needs, identify barriers to care, and evaluate available resources.

• Recognize and facilitate connections to providers, specialists, and community resources.

• Offer psychosocial and resource assistance regarding employment, education, housing, food, independent living, justice, and foster care needs.

• Coordinate efforts between members, families, caregivers, and care provider teams.

• Track care plan objectives and member status, collaborating on necessary revisions or further assessments.

• Gather, record, and maintain member and care management data to ensure regulatory compliance.

• Conduct on-site visits when deemed appropriate.

• Instruct care managers and members/caregivers on processes, provider guidelines, referrals, and healthcare benefits.

• Execute other assigned tasks while adhering to established policies and standards.

⛳️ Requirements

• Bachelor’s degree with 2–4 years of relevant experience.

• Alternatively, equivalent experience gained through applicable knowledge, responsibilities, scope, and skills reflective of the position level.

• If possessing clinical licensure, graduation from an accredited nursing school is required.

• Current state clinical license is preferred.

• Availability to work Monday–Friday, from 8am to 5pm.

• Previous experience in foster care is highly preferred.

• Capability to work remotely from any location within Missouri.

• Perform telephonic outreach to foster care members and guardians.

• Familiarity with care management, healthcare access, care coordination, member assessments, personalized care plans, and community resources.
