Care Navigator

This is a fully remote position, open to applicants in United States.

📋 Description

• Act as the primary point of contact for patients or their families contacting Sailor Health.

• Assist older adults and caregivers throughout the intake process.

• Provide empathy, clarity, professionalism, and emotional support during initial interactions.

• Coordinate patient care and facilitate the navigation of the care journey.

• Pair patients with the appropriate therapist.

• Ensure patients feel confident and understood throughout the intake and care process.

• Take initiative and contribute to the development of key processes as part of a small, dynamic team.

⛳️ Requirements

• Must be based in the US for remote work.

• Strong empathetic and clear communication skills.

• Proficient in active listening and managing difficult or emotional conversations.

• Highly organized with excellent attention to detail.

• Capability to handle multiple patients at various stages.

• Comfortable learning Google Workspace, EHR platforms like Healthie, and CRMs such as HubSpot.

• Ability to quickly adapt to new software.

• A genuine interest in mental health and healthcare accessibility.

• Prior experience is appreciated but not mandatory; relevant experience includes healthcare, customer service, or administrative support.

• Must be comfortable working during Eastern Time Zone hours.

• Availability to work one of the specified shifts: Monday–Friday, Wednesday–Sunday, or Saturday–Wednesday, from 9 a.m.–6 p.m. ET or 12 p.m.–9 p.m. ET.
