This is a fully remote position, open to applicants in Canada.
• Act as the primary contact for patients and their families.
• Assist older adults and caregivers through the intake procedure.
• Deliver patient support, care coordination, and emotional assistance.
• Aid patients in finding the appropriate therapist.
• Support patients during their transition into care.
• Ensure patients feel supported, comprehended, and reassured.
• Take initiative and contribute to the development of key processes within a small, agile team.
• Must be located in Canada.
• Possess strong empathy and effective communication skills.
• Demonstrate active listening and the capability to manage challenging or emotional discussions.
• Highly organized and detail-oriented.
• Ability to oversee multiple patients at various stages.
• Comfortable with learning Google Workspace, EHR platforms such as Healthie, and CRMs like HubSpot.
• Quick to adapt to new software.
• Interest in mental health and accessibility to healthcare.
• Experience in healthcare, customer service, or administrative support is a plus but not mandatory.
• Willing to work hours aligned with the Eastern Time Zone.
