Care Navigator

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Design, evaluate, and manage care coordination activities tailored to individual member needs.

• Create or assist in the development of customized care plans/service plans.

• Provide education to members and their families/caregivers regarding services, benefit alternatives, care options, referrals, healthcare coverage, and provider guidelines.

• Assess member requirements, obstacles to care, available resources, and develop strategies to achieve optimal outcomes.

• Identify necessary providers, specialists, and community resources for effective care.

• Offer psychosocial and resource assistance concerning employment, education, housing, nutrition, participant direction, independent living, justice, and foster care.

• Facilitate communication between members/families/caregivers and care provider teams to ensure prompt access to care and services.

• Track progress toward care plan objectives and monitor changes in member status or condition.

• Collaborate with healthcare providers to adjust care plans and refer members for further evaluations in care management as necessary.

• Gather, document, and preserve member information and care management activities to ensure regulatory compliance.

• Conduct on-site assessments when appropriate to evaluate member needs and work with providers or resources.

• Carry out other responsibilities as assigned by the leadership.

⛳️ Requirements

• Bachelor’s degree.

• 2–4 years of relevant experience.

• If holding clinical licensure, graduation from an accredited School of Nursing is required.

• Equivalent experience gained through relevant knowledge, responsibilities, scope, and skills may fulfill this requirement.

• Current state clinical license is preferred.

• Missouri LPN license is strongly preferred.

• Capability to work Monday–Friday, 8 am–5 pm.

• Ability to work remotely from any location within Missouri.
