This is a fully remote position, open to applicants in United States.
β’ Oversee the daily execution of a specified care model workstream, ensuring tasks and milestones are delivered as scheduled.
β’ Create and sustain task-level work plans; monitor actions, responsible parties, dependencies, and due dates.
β’ Facilitate collaboration among functional contributors to ensure timely completion of workstream deliverables.
β’ Identify and address risks and issues at the workstream level; suggest options for mitigation and assess timing effects.
β’ Raise awareness of risks, issues, or delays affecting the overall care model critical path.
β’ Deliver precise and timely status updates and contributions to integrated workstream reports.
β’ Maintain documentation for the workstream, including action logs and supporting materials.
β’ Ensure operational excellence, meticulous attention to detail, and accountability for designated outcomes.
β’ Convert care model strategy and guidance into detailed task-level plans.
β’ Act as an operational partner to the Workstream Project Manager and Senior Care Model leadership.
β’ 3β5 years of experience in healthcare operations, care model implementation, clinical programs, or similar operational roles that support care delivery or value-based initiatives.
β’ Practical experience in managing workstreams or projects, including planning, execution, tracking, and follow-up within a defined scope.
β’ Proven ability to coordinate cross-functional contributors, manage dependencies, and drive deliverables to completion without direct authority.
β’ Experience in identifying and managing risks and issues, including the ability to propose mitigation strategies and escalate impacts when necessary.
β’ Familiarity with producing detailed status reports and maintaining work artifacts such as action logs, timelines, and documentation.
β’ Understanding of healthcare delivery systems and regulatory considerations.
β’ Experience in supporting care model, quality, or operational improvement initiatives is preferred.
β’ Strong proficiency in developing detailed work plans, tracking actions, responsible parties, and due dates, and driving towards closure.
β’ Ability to identify workstream-level risks and issues, evaluate impacts, and suggest practical mitigation strategies.
β’ Sound judgment to escalate risks or delays that affect broader timelines, with clear communication of impacts.
β’ Capability to provide accurate and timely status updates while maintaining action logs, notes, and workstream artifacts.
β’ Excellent written and verbal communication skills.
β’ Strong organizational abilities, operational diligence, attention to detail, and accountability for designated outcomes.
β’ Candidate can reside anywhere within the US.
β’ A Bachelor's degree in healthcare administration, nursing, public health, or a related field is preferred, or equivalent experience.
