Care Manager, RN

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Oversee care for members with intricate, high-risk requirements

• Perform assessments and screenings

• Create, evaluate, and facilitate complex care management initiatives

• Develop tailored care plans and educate members and their families

• Assess member needs, barriers to care access, and social determinants of health

• Collaborate with healthcare providers, specialists, caregivers, and community services

• Identify challenges and obstacles to care and implement care management strategies

• Coordinate person-centered care and services

• Monitor member status, changes in condition, and progress of care plans

• Offer resource support for members and Care Managers

• Ensure timely access to required care and services

• Conduct outreach through telephone, digital platforms, home visits, and other locations

• Gather, document, and maintain member information and care management activities

• Educate members and families about disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits

• Assist fellow Care Managers by addressing inquiries and providing guidance on complex cases

• Offer leadership insights on opportunities to enhance care quality and cost-effectiveness

• Carry out other assigned responsibilities while adhering to policies and standards

⛳️ Requirements

• Must possess maternity care management experience

• Residency in North Carolina is required

• Experience with the pediatric population is mandatory

• Degree from an accredited nursing program or a Bachelor's degree in Nursing

• 2–4 years of relevant experience

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is required
