Care Manager, RN

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Deliver comprehensive, high-risk care management for pediatric patients.

• Perform assessments and screenings.

• Create ongoing care and service plans.

• Assess member needs, obstacles to care, and social determinants of health.

• Work collaboratively with providers, specialists, caregivers, and community resources.

• Identify issues and barriers to care and implement interventions.

• Coordinate person-centered care and services.

• Monitor member status, changes in condition, and progress of care plans.

• Provide resource support for services related to employment, housing, independent living, justice, and foster care.

• Ensure timely access to necessary care and services.

• Conduct outreach via telephone, digital methods, home visits, or other locations.

• Gather, document, and maintain member information and care management activities for regulatory compliance.

• Educate members and their families/caregivers on disease processes, care gaps, care options, referrals, and healthcare benefits.

• Assist fellow Care Managers by addressing inquiries and offering guidance on complex cases.

• Provide feedback to leadership regarding opportunities to enhance care and quality delivery.

• Carry out additional assigned duties and adhere to policies and standards.

⛳️ Requirements

• Experience in pediatric care management.

• Valid Registered Nurse (RN) license.

• Applicants must be residents of North Carolina.

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing.

• 2–4 years of relevant experience.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is mandatory.
