
Care Manager β Nurse Practitioner

This is a fully remote position, open to applicants in Alabama, +19 more states.
β’ Oversee the comprehensive care management and quality of services provided to participants.
β’ Evaluate assessments and documentation from field staff to ensure quality and address any deficiencies.
β’ Engage in care planning meetings and serve as a vital member of the interdisciplinary team (IDT).
β’ Coordinate holistic, member-focused care plans and the continuum of care.
β’ Organize, manage, and authorize suitable community-based, long-term care, skilled, and support services.
β’ Mentor, train, and lead competency sessions for field staff.
β’ Supervise field staff documentation, interventions, services, utilization, and member feedback.
β’ Collaborate with physicians, IDT members, facilities, discharge planners, home care coordinators, and specialty services.
β’ Record services in accordance with CLHC standards and federal/state regulations.
β’ Assess the effectiveness of care plans and adjust them based on changes in members' health and objectives.
β’ Stay informed about health issues, procedures, evidence-based clinical guidelines, medications, and practice standards.
β’ Suggest enhancements to services, programs, policies, and procedures.
β’ Adhere to disciplinary policies and consult Human Resources as necessary.
β’ Travel between sites and locations as needed.
β’ Perform additional duties as assigned.
β’ Completed a nurse practitioner education program recognized by the New York State Education Department (NYSED).
β’ NP Board Certification required.
β’ At least two (2) years of experience in Care Planning.
β’ Minimum of two (2) years of administrative experience in a management role within a certified home health agency (CHHA), long-term home health care (LHCSA), acute care, medical-surgical, and/or critical care settings; nursing home experience or diagnostic & treatment clinic background preferred.
β’ Experience in Customer Service is essential.
β’ Familiarity with long-term care insurance is advantageous.
β’ At least one (1) year of experience working with a frail or elderly population, or appropriate PACE organization training upon hiring if lacking one year of experience.
β’ Supervisory experience is preferred.
β’ Current and unrestricted licensure and registration in New York State is mandatory.
β’ Legally authorized to practice in the state where the duties are performed.
β’ Must be medically cleared for communicable diseases and have all vaccinations up-to-date before engaging in direct participant contact.
β’ Bilingual candidates are preferred.
β’ Capable of meeting the specified physical, cognitive, visual, hearing, mobility, lifting, and fine motor requirements.
β’ Available to work from 8:00 AM to 5:00 PM, 40 hours per week.
β’ Willing to commute within a 2-hour range of the remote location.
