This is a fully remote position, open to applicants in California.
• Develop, evaluate, and implement intricate care management activities primarily focused on mental and behavioral health requirements.
• Deliver high-quality, cost-efficient healthcare outcomes through tailored care plans and education for members and their families.
• Assess member needs through phone consultations or home visits, recommending or facilitating appropriate care and service plans.
• Engage in telephonic, digital, home, and other outreach visits to evaluate member needs and collaborate with available resources.
• Create ongoing care plans for members with elevated acuity levels.
• Identify providers, specialists, and community resources for mental health and substance use disorder treatment.
• Coordinate efforts among members, families, caregivers, community resources, and care provider teams.
• Monitor care plans, member status, outcomes, treatment side effects, complications, and clinical symptoms.
• Facilitate care coordination and ensure timely access to necessary care and services.
• Collect, document, and uphold member information and care management activities to ensure regulatory compliance.
• Educate members and families regarding procedures, provider instructions, treatment options, referrals, and healthcare benefits.
• Provide leadership feedback on opportunities for enhancing care and quality delivery.
• Perform additional duties as assigned.
• A Master's Behavioral Health Professional license in California is required.
• Preference for candidates residing in California.
• Must be available to work during PST hours.
• A Master's degree in Behavioral Health or Social Work, or a degree from an accredited nursing program is required.
• 2–4 years of relevant experience is necessary.
• Must hold a Licensed Master's Behavioral Health Professional designation (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN, depending on state contract requirements, with behavioral health experience.
• California licensure is mandatory.
