This is a fully remote position, open to applicants in New Jersey.
• Evaluate client service/grid notes to ensure that documentation aligns with regulatory standards.
• Keep client and staff records accurate and up-to-date.
• Facilitate clinical and supervisory meetings, conduct annual evaluations, and create staff development plans.
• Engage in staff meetings and client ISP meetings while advocating for the needs of clients.
• Create personalized goals, task analyses, and interventions.
• Complete assessments, progress reports, and AENs as necessary.
• Supervise staffing patterns, manage scheduling, and handle PTO/payroll requests.
• Ensure that client-specific trainings are completed and that OnTarget goal entries are accurate.
• Address client/guardian grievances and finalize incident reports within mandated timelines.
• Foster community relationships, represent Abound Health NJ at various events, and assist with resource access.
• Offer support, services, advocacy, and guidance to individuals benefiting from New Jersey Waiver Services.
• Guarantee high-quality service delivery, maintain compliance, and support both clients and direct care staff.
• Capability to manage shifting priorities and operate effectively in a dynamic environment.
• Exceptional communication and interpersonal skills when interacting with clients, families, staff, and community partners.
• Proficiency in multitasking, problem-solving, and upholding professionalism under pressure.
• Strong knowledge of ISP implementation, goal writing, task analysis, and Medicaid/third-party billing.
• Ability to collaborate within a team setting while maintaining therapeutic relationships.
• Keen attention to detail, accuracy, and organizational skills.
• Alignment with the mission, vision, and values of Abound Health NJ.
• One of the following credentialing pathways: Master’s degree + 1 year of experience directly working with persons with disabilities; Bachelor’s degree + 2 years of experience; Associate’s degree or 60 credits + 4 years of experience; High school diploma/GED + 5+ years of experience.
