This is a fully remote position, open to applicants in Oklahoma.
• Conduct outreach to members via phone regarding next steps in their care plans, community or health plan resources, scheduling inquiries, concerns, and ongoing education.
• Link members with relevant health plan and community resources.
• Utilize a solid understanding of assigned health plan activities and available resources.
• Offer front-line assistance for member and provider inquiries, requests, and concerns.
• Clarify care plan procedures and protocols to members.
• Assist with member onboarding and manage daily administrative tasks.
• Dispatch welcome letters, correspondence, and educational materials related to programs.
• Record and maintain non-clinical member documentation in line with state and regulatory requirements.
• Provide necessary records to providers as needed.
• Make referrals to address Social Determinants of Health needs.
• Adhere to all policies and standards.
• Carry out other assigned duties.
• Candidates must be residents of Oklahoma.
• High School diploma or GED is required.
• 1-2 years of relevant experience is preferred.
• Experience in healthcare is ideal.
• Previous experience in a call center is beneficial.
• Proficient in computer usage.
• Highly organized and self-driven.
• Exceptional customer service and communication abilities are essential.
• Must be available to work Monday through Friday, from 8:00 AM to 5:00 PM, with evening and weekend availability as required by program and client requirements.
