This is a fully remote position, open to applicants in Colorado, +9 more states.
• Facilitate care coordination for members enrolled in Medicaid, Medicare Advantage, and/or Dual Eligible Special Needs Plan (DSNP).
• Establish trusted relationships and foster a positive experience for members.
• Assist members in obtaining suitable care through health plans, medical providers, pharmacies, vendors, and community-based organizations.
• Evaluate members' medical and social requirements, including Social Determinants of Health.
• Conduct outreach to encourage, facilitate, and inform members about program benefits.
• Perform telephone and/or text assessments and collaborate with members to develop care plans.
• Assess individual care needs and relay medical information to healthcare professionals.
• Manage a caseload and ensure prompt communication with members.
• Coordinate care with providers and arrange appointments as necessary.
• Inspire members to take an active role in their health and overall well-being.
• Organize and complete correspondence according to established workflows.
• Accurately document actions within a care management platform.
• Conduct extensive outreach to members, families, providers, and other stakeholders.
• Advocate for, support, and motivate members in managing their health and healthcare.
• Maintain ongoing relationships with members, families, providers, and external partners.
• Active and unrestricted Licensed Vocational Nurse / Licensed Practical Nurse (LVN/LPN) or Registered Nurse (RN) license.
• Minimum of 2 years of experience as a care coordinator, case manager, community health worker, pharmacy technician, or social worker.
• Must reside in Texas, Louisiana, Mississippi, Arkansas, Missouri, Colorado, Oklahoma, Kansas, Utah, Nebraska, or Wyoming.
• A dedicated home office setup for remote work.
• At least 1 year of experience in a remote work environment.
• Experience with Medicaid and/or Dual Eligible patients.
• Strong interpersonal, verbal and written communication, and problem-solving abilities.
• Proven experience interacting with customers via phone and text messaging.
• Background in documenting case notes within a care management or electronic health records platform.
• Familiarity with motivational interviewing techniques.
• Preferred experience with Apple computers, such as MacBook.
• Bilingual proficiency in Spanish is preferred.
• Knowledge or experience with post-discharge follow-up is preferred.
• Knowledge or experience in chronic disease management is preferred.
• Capability to work effectively in a multicultural environment with diverse populations.

• Skill in maintaining positive working relationships with members, families, providers, and external partners.
