This is a fully remote position, open to applicants in Tennessee.
• Engage in outreach efforts to inspire, guide, and inform members about the advantages of available programs.
• Perform assessments of members’ conditions and collaborate with them to create a care plan tailored to their objectives.
• Assessments may be conducted via telephone and/or text communication.
• Evaluate the specific care requirements of each member and relay medical information to healthcare professionals.
• Oversee a caseload of members to guarantee prompt communication with each individual.
• Facilitate care coordination with healthcare providers and arrange appointments as necessary.
• Encourage members to actively participate in their health and overall wellness.
• Identify and assist in addressing needs related to Social Determinants of Health.
• Organize and finalize correspondence in line with established workflows.
• Diligently document all actions taken within a care management platform.
• Execute a high volume of outreach efforts to members, families, providers, or other relevant parties as required to effectively fulfill the role.
• Possession of an active and unrestricted Licensed Practical Nurse (LPN) or Registered Nurse (RN) license.
• Proficiency in both Spanish and English is essential.
• A minimum of 2 years of experience in roles such as care coordinator, case manager, community health worker, pharmacy technician, or social worker.
• Must reside in Tennessee.
• A dedicated home office setup for remote work is required.
• At least 1 year of experience working remotely.
• Familiarity with Medicaid and/or Dual Eligible patients is preferred.
• Strong interpersonal, verbal and written communication, and problem-solving skills are necessary.
• Experience in customer interactions via phone and text messaging.
• Proficiency with Apple computers (e.g., MacBook) is required.
• Experience in documenting case notes in a care management or electronic health record platform is essential.
• Knowledge of motivational interviewing techniques is advantageous.
