Care Coordinator III

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Assist the senior care management team and the teams dedicated to members.

• Organize care to guarantee that services are provided and that continuity of care and member satisfaction are maintained.

• Act as a communication bridge among care managers, healthcare providers, community providers, partners, members, and caregivers.

• Carry out member outreach via phone calls or home visits.

• Review care plans/service plans, outline next steps, share resources, address questions and concerns, and provide ongoing education.

• Oversee care activities based on care/service plans and adjust for any changes or progress.

• Address inquiries, requests, and concerns from members and providers.

• Acquire knowledge of care management services and handle complex or escalated issues.

• Communicate with care managers, practitioners, and others to facilitate member services.

• Conduct service assessments/screenings for members with complex needs and document their care requirements.

• Maintain and document member records in compliance with state and regulatory standards.

• Triage, modify, and escalate complex requests to management as needed.

• Adhere to contractual, regulatory, and organizational guidelines and policies.

• Provide education regarding benefits and available resources.

• Support training and development initiatives.

• Execute other assigned responsibilities.

⛳️ Requirements

• Candidates must reside in one of the following Illinois counties: Cook, DuPage, Grundy, Kane, Kankakee, Kendall, Lake, McHenry, or Will.

• Availability from Monday to Friday, during the hours of 8am to 5pm Central time zone.

• A Bachelor’s degree in nursing, social sciences, social work, or a related field in human services/social services for the Illinois Youth Care plan.

• At least one (1) year of supervised clinical experience in a human-services area.

• A High School diploma or GED is required.

• 2–4 years of relevant experience is preferred.

• Residency in Illinois is necessary.

• Capability to conduct member outreach either by phone or through home visits.

• Familiarity with care management services and experience managing complex or escalated issues.

• Ability to perform service assessments/screenings and document member care needs accurately.

• Proficiency in maintaining records as per state and regulatory requirements.

• Skill in identifying needs and making referrals to Care Managers, community-based organizations, and Disease Managers.
