This is a fully remote position, open to applicants in Minnesota.
• Independently manage a designated caseload of members.
• Develop personalized care plans.
• Coordinate the delivery of sanctioned medical and social services.
• Educate members about their Medicare and Medicaid benefits.
• Act as a reliable primary contact for members, addressing and resolving inquiries and concerns.
• Collaborate with physicians, providers, and county financial entities regarding care plans.
• Provide updates on the progress of members.
• Conduct telephonic and in-home assessments for members.
• Organize and manage work according to member availability and regulatory requirements.
• Travel within the community to meet with members in their residences.
• Execute other duties as assigned.
• Must reside in Minnesota and support membership within the state.
• RN: Hold an RN diploma or degree in Nursing (Associate's or Bachelor's) with at least 2 years of experience in community-based services, OR possess a Bachelor's degree in Nursing with a Public Health Certification and a minimum of 1 year experience in community-based services.
• RN: 3–5+ years of experience beyond the degree.
• SW: Bachelor's degree in Social Work.
• SW: 3–5+ years of experience beyond the degree, including 1–2+ years in community-based services.
• Active, unrestricted RN license or SW license in Minnesota.
• Preferred: MnCHOICES Certified Assessor Training (MNCAT) or ability to obtain within 21 days of employment.
• Public Health Certification preferred but not mandatory.
• Experience in home and community-based services or case management.
• Experience working with individuals with disabilities and senior populations.
• Comfortable working with diverse, low-income communities.
• Extensive knowledge of Medicare and Medical Assistance programs.
• Excellent interpersonal skills with the ability to simplify medical terminology for non-medically trained members.
• Capable of working independently, collaborating as a team player, shifting priorities, and adjusting tasks as needed.
• Strong clinical and documentation skills.
• Exceptional innovative problem-solving abilities.
• Outstanding organizational skills.
• Willingness to travel within Medica's Service Area.
• Reliable means of transportation.
• A designated private area in the home for a home office setup.
• General comfort with technology and proficiency in Microsoft Office Suite.
• Must be legally authorized to work in the United States; Medica does not provide work visa sponsorship.
• 4 or 5-day work week.
