Care Coordinator II, Foster Care

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Assist with care management tasks and support designated member-care teams

• Ensure that services are provided by healthcare professionals and affiliated partners

• Foster continuity of care and enhance member satisfaction

• Conduct outreach to members via phone calls or home visits

• Record care and service activity plans

• Discuss care plans, next steps, available resources, questions, concerns, and ongoing education with members

• Coordinate care activities with healthcare and community providers, partners, members, and caregivers

• Address inquiries, requests, and concerns from members and providers

• Communicate with care managers, practitioners, and others to facilitate member services

• Assist with service assessments and screenings

• Maintain and document member records, distributing them to providers as necessary

• Adhere to contractual, regulatory, and organizational standards and policies

• Provide education regarding available benefits and resources

• Carry out other duties as assigned

⛳️ Requirements

• High School diploma or GED

• 1–2 years of relevant experience

• Previous experience with the Foster Care population is highly preferred

• Availability to work Monday–Friday, 8am–5pm

• Capability to work remotely from anywhere within the state of Missouri

• Ability to provide telephonic support to Foster Care children

• Competence in conducting member outreach through phone calls or home visits

• Skill in coordinating care with healthcare and community providers, partners, members, and caregivers

• Proficiency in identifying needs and referring to Care Managers, community-based organizations, and Disease Managers

• Capacity to document and maintain member records in accordance with state and regulatory requirements
