Care Coordinator

This is a fully remote position, open to applicants in Canada.

📋 Description

• Act as the initial point of contact for patients and their families.

• Assist older adults and caregivers during the intake process.

• Offer patient support, care coordination, and emotional assistance.

• Aid patients in navigating their entry into care.

• Connect patients with the appropriate therapist.

• Provide support to patients throughout their intake and care-navigation experience.

• Ensure older adults and families feel supported, understood, and reassured.

⛳️ Requirements

• Applicants must reside in Canada.

• Availability for full-time work.

• Strong empathetic and clear communication skills.

• Proficient in active listening and managing challenging or emotional conversations.

• Highly organized with a keen attention to detail.

• Ability to handle multiple patients at various stages.

• Comfortable with learning Google Workspace, EHR platforms like Healthie, and CRMs such as HubSpot.

• Quick adaptability to new software.

• Interest in mental health and access to healthcare.

• Willingness to work during Eastern Time Zone hours.

• Open to working one of the specified shifts: 9a–6p ET or 12p–9p ET, according to the outlined weekly schedules.

• Experience in healthcare, customer service, or administrative support is preferred but not mandatory.
