Care Coordinator

This is a fully remote position, open to applicants in Texas.

📋 Description

• Collaborate with administration, Care Managers, patients, and their families to achieve healthcare objectives and ensure effective communication.

• Assist with non-clinical responsibilities and provide support to clinicians, case managers, and team members concerning patient care.

• Act as a resource and educational reference for patients, families, providers, and staff.

• Conduct telephonic follow-ups with members, including those post-hospital discharge and those managing chronic conditions.

• Complete designated Care Coordination tasks.

• Create tasks for the assigned RN Case Manager.

• Guide patients and families to the appropriate resources, including community services.

• Support, under the guidance and supervision of the RN Case Manager, with member goals, education, resource access, and follow-up care arrangements.

• Ensure high-quality service in line with organizational standards.

• Assist with data entry, faxing, filing, and various clerical tasks.

• Manage inbound and outbound calls, gathering information as requested by licensed staff.

⛳️ Requirements

• Availability to work a shift from 9 AM to 6 PM CST or 10 AM to 7 PM CST is required.

• A minimum of 1 year of relevant experience in a healthcare-related position.

• High school diploma or GED is mandatory.

• Strong organizational and time management abilities.

• Exceptional verbal and written communication skills.

• Proficiency in professional and efficient telephonic communication.

• Competence in Microsoft Office Suite (Excel, Word, Outlook, PowerPoint).

• Capacity to multi-task effectively in a fast-paced environment.
