This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct telephonic outreach to enroll and engage patients selected for the clinical program.

• Serve as the primary contact for patients who are enrolled.

• Collaborate with the Nurse Care Manager to create and execute care plans.

• Ensure smooth transitions of care between various settings and dialysis centers.

• Revise patient care plans to improve care delivery and coordination.

• Perform patient assessments to identify social, economic, and environmental factors affecting health.

• Suggest solutions for gaps in care and connect members with housing, transportation, food security, and community support resources.

• Assist members in navigating health and social services as well as community organizations.

• Advocate for members by recognizing barriers to care and addressing disparities in health access and outcomes.

• Communicate effectively with patients, families, caregivers, and healthcare teams.

• Document interactions, assessments, and care plans in compliance with organizational and regulatory standards.

• Provide educational resources and solutions to empower patients and their families.

• Perform additional duties as assigned, including cross-market coverage.


⛳️ Requirements

• A minimum of 3 years of experience in healthcare coordination, community health work, or a related healthcare field.

• Proficiency in a team-based approach to care.

• Strong knowledge of community resources.

• Exceptional ability to prioritize tasks and switch between them to meet the needs of patients and the care team.

• Self-motivated and proactive in addressing issues professionally and creatively.

• Excellent communication and teaching skills.

• Intermediate proficiency in Microsoft Office Suite, including Outlook and Teams.

• Ability to work effectively in a predominantly remote setting.

• Minimum internet download speed of 25 Mbps and upload speed of 10 Mbps.

• A wired internet connection via Cable, Fiber, or DSL; a hardwired connection to the device is preferred.

• Must work from a designated area free from ongoing interruptions to safeguard member PHI / HIPAA information.
