This is a fully remote position, open to applicants in Maryland.

📋 Description

• Facilitate patient care by ensuring access to appropriate resources and programs and assisting with referrals and provider appointments.

• Conduct outbound calls and manage incoming calls from patients, providers, and Village resources.

• Execute non-clinical screenings and pathways under the guidance of the Manager; escalate issues to the Integrated Care Nurse as necessary.

• Support the clinical team in progressing patients through the continuum of care by completing assigned tasks.

• Identify qualified resources and collaborate with Integrated Care Nurses, dialysis clinics, health plans, and other stakeholders to assist members in enrolling in programs.

• Communicate directly with patients in a prompt and professional manner.

• Ensure accurate, complete, and confidential documentation is maintained.

• Exhibit excellent two-way oral and written communication skills.

• Act in accordance with DaVita’s mission and values to provide high-quality patient care.


⛳️ Requirements

• A high school diploma or GED is required.

• A minimum of 2 years of experience in a professional office setting.

• Capability to handle confidential information with discretion and sound judgment.

• Strong verbal communication abilities.

• Proficiency in Microsoft Office, Excel, and Outlook.

• Experience with Electronic Medical Records is preferred.

• A commitment to service excellence and patient satisfaction.

• Ability to work independently while effectively collaborating with the team.

• A quiet, private home workspace with reliable high-speed internet is essential.

• 1 to 3 years of clinical medical office experience is preferred.

• CNA or MA experience is an advantage.

• Previous experience in care coordination (referrals, care management, etc.) is preferred.

• Familiarity with navigating insurance payers is preferred.

• Experience in direct patient care or support is preferred.

• Prior experience in a medical office environment is preferred.
