Care Coach

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Oversee members' health conditions through telephonic strategies, methodologies, and techniques.

• Identify and address obstacles that impede effective care.

• Monitor patient care via assessments, data analysis, discussions with members, and active care planning.

• Ensure patients make progress towards their desired health outcomes.

• Direct members and their families to vital health and social service resources.

• Make informed decisions regarding work strategies and priorities while adhering to established guidelines, procedures, regulations, and professional standards.

• Engage in HireVue interviews as part of the recruitment process.

• Occasionally travel to Humana offices for training or meetings.

⛳️ Requirements

• Bachelor's degree in a health or social service-related discipline or LPN licensure with the capability to obtain a license in Illinois.

• A minimum of 2 years of case management experience.

• Proficiency in Microsoft Word, Excel, and Outlook.

• Experience with electronic information applications and software programs.

• Preferred residence in Illinois.

• Availability to work Monday through Friday, from 8:00am to 5:00pm CST, with occasional scheduling flexibility.

• Internet service with a minimum of 25 Mbps download and 10 Mbps upload speed.

• Capability to work from a dedicated environment without constant interruptions to safeguard member PHI/HIPAA information.
