Care Coach

This is a fully remote position, open to applicants in Alabama, +13 more states.

📋 Description

• Oversee the health concerns of members by employing telephonic strategies, methodologies, and techniques.

• Recognize and address obstacles that impede effective care delivery.

• Perform thorough assessments and consistently track patient care and progress towards desired outcomes.

• Create and implement active care plans informed by assessments, data, and discussions with members.

• Assist members and their families in accessing vital healthcare and community resources.

• Adhere to professional concepts, standards, regulations, strategies, operational standards, and established procedures.

• Make decisions related to work methods and priorities under the guidance of management.

• Report directly to the Manager of Care Management.

⛳️ Requirements

• A bachelor's degree in a health-related discipline, active Illinois LPN licensure, or the capability to acquire an Illinois LPN license.

• At least 2 years of experience in case management, medical case management, care coordination, or healthcare support.

• Extensive proficiency in Microsoft Office applications, including Word, Excel, and Outlook.

• Bilingual in English and Spanish is preferred.

• Certification as a Case Manager (CCM) is preferred.

• A Master's Degree is preferred.

• Experience in managed care is preferred.

• Previous experience with Medicare and Medicaid recipients is preferred.

• Familiarity with electronic case note documentation and various computer applications/systems is preferred.

• Experience in health promotion, coaching, and wellness is preferred.

• Knowledge of community health and social service agencies, as well as community resources, is preferred.
