This is a fully remote position, open to applicants in United States.
• Act as a vital clinical connection between care services and community-based support for new mothers and their infants.
• Offer guidance during pregnancy and throughout the postpartum period.
• Conduct telemedicine appointments at designated times during the postpartum phase.
• Identify unmanaged clinical issues and educate members on intricate clinical subjects.
• Evaluate Urgent Maternal Warning Signs and assess high blood pressure readings.
• Assist members in navigating to the most suitable healthcare settings.
• Review blood pressure metrics and Edinburgh Postnatal Depression Scale (EPDS) scores.
• Follow up on any irregular values that have not yet been escalated.
• Collaborate clinically with members’ obstetric providers or primary care physicians.
• Document visit summaries and escalate unmanaged clinical needs to the Care team.
• Provide training to Maternity Navigators on prenatal and postpartum topics.
• Integrate community insights into the development of the Nadia Care program.
• Encourage collaboration between Maternity Navigators and clinicians through case discussions, knowledge sharing, and care team huddles.
• Report directly to the Senior Manager of Care Delivery.
• Valid compact state Registered Nurse (RN) license in TN or IN, with a desire to obtain licensure in additional compact states.
• At least 5 years of experience in prenatal and postpartum care in outpatient pediatrics, OB/GYN, maternal-fetal medicine, acute care, or a related environment.
• Native or bilingual proficiency in speaking, writing, and reading Spanish.
• Experience in providing clinical information, patient education, and medical guidance in Spanish.
• Outstanding interpersonal and communication abilities.
• Capacity to deliver compassionate, empathetic, and culturally-informed care.
• Highly organized and solution-focused.
• Ability to thrive in a fast-paced and evolving setting.
• Comfortable building strong relationships with patients virtually.
• Tech-savvy or quick to learn systems for documenting care and communicating with members, including AthenaOne, Spruce, and MacOS.
