This is a fully remote position, open to applicants in Canada.
• Deliver medical and patient support to patients, caregivers, and healthcare professionals.
• Inform patients and providers about diseases, treatments, and Patient Support services utilizing approved materials.
• Perform patient evaluations and address inquiries related to programs, diseases, and products.
• Serve as a communication link between patients, families, healthcare providers, pharmacies, and clinics concerning medication access and treatment logistics.
• Offer continuous adherence support and direct clinical inquiries beyond program materials to healthcare providers.
• Record patient information and interactions in electronic health records.
• Review and amend patient files throughout the patient journey.
• Adhere to case-management protocols, which include data collection, clinical monitoring, follow-up, therapy disruption/discontinuation monitoring, compliance, and reporting of adverse events.
• Work collaboratively with reimbursement specialists and physicians to secure therapy funding and explore alternative funding options.
• Organize and monitor patient services according to KPIs to facilitate therapy access.
• Engage in specialized therapy training, pharmaceutical meetings, patient gatherings, conferences, and patient advocacy events.
• Keep in touch with the manager, team, and client regarding patient case management.
• Help train new Nurse Case Managers and provide feedback on protocols, databases, processes, and inter-departmental interactions.
• Assist in the development and execution of disease-management programs, which include clinical requirements, patient materials, compliance parameters, needs assessments, and protocols.
• Report adverse events and severe adverse events in line with program protocols.
• Undertake additional duties as assigned and be available for standby/on-call work when necessary.
• Bachelor’s degree or licensed nursing diploma (IA), advanced studies, or specific experience in office management is preferred.
• A minimum of five (5) years of nursing experience.
• Active registration as a Licensed Nurse (RN or RPN/LPN) from the College of Nurses in the province of employment is mandatory.
• At least two (2) to five (5) years of experience in healthcare, pharmaceuticals, insurance, or related fields.
• Preferred experience in healthcare and case management within medical/hospital environments, insurance, or the bio-pharmaceutical industry.
• Proficiency in French and/or English may be required, depending on the program and geographic area.
• Experience in hematology and/or nephrology is a plus.
• Strong focus on patient-centered care.
• Capacity to multitask and perform well under pressure.
• Exceptional skills in customer service, telephone etiquette, communication, problem-solving, teamwork, negotiation, analytical thinking, mathematics, organization, interpersonal relations, leadership, and presentations.
• Ability to comprehend and convey medical information effectively.
• Competence in internet research and computer skills.
• Proficiency in Microsoft Word, Excel, PowerPoint, and other Office applications.
• Capability to work independently and consistently meet deadlines.
• Willingness to travel at least 15% of the time each year.
