This is a fully remote position, open to applicants in United States.
• Oversee and coordinate healthcare services to deliver comprehensive, patient-focused care.
• Evaluate patients’ medical requirements and create tailored care plans.
• Advocate for patients’ rights, choices, and access to suitable services.
• Track patient progress, adherence to care plans, and the effectiveness of interventions.
• Modify interventions as needed.
• Accurately and promptly document assessments, care plans, and outcomes.
• Prepare reports on patient conditions, care results, and resource usage.
• Educate patients regarding medications, treatment strategies, and self-care techniques.
• Offer emotional support and counseling to both patients and their families.
• Encourage preventive care and wellness through health education and lifestyle advice.
• Ensure compliance with URAC standards, CMSA Standards of Practice, relevant regulations, and Valenz policies.
• Collaborate with internal teams to uncover opportunities for health plan coverage savings.
• Perform additional responsibilities as assigned.
• Work remotely utilizing company-provided equipment while maintaining a secure workspace and compliant technological environment.
• Current, unrestricted Registered Nurse (RN) license in the state of residence.
• Compact License (NLC) accepted for those residing in a compact state.
• A minimum of 3 years nursing experience in an acute care environment.
• Capability to thrive in a deadline-oriented setting.
• Strong attention to detail and excellent organizational abilities.
• Critical thinking skills and sound judgment in complex scenarios.
• Ability to escalate issues appropriately when necessary.
• Exceptional communication and interpersonal skills.
• Collaborative mindset in care coordination.
• Reliable internet connection is essential.
• A quiet workspace that is free from distractions.
• Compliance with company security protocols, including VPN usage, secure passwords, and company-approved devices/software.
• Must be based in the US and located in an area where work can be conducted effectively in line with company policies such as HIPAA.
• Availability to work until at least 4:00 PM Pacific Time.
• Experience in case management is preferred.
• Bilingual proficiency in Spanish is advantageous.
• Relevant certifications such as Certified Case Manager (CCM) or Chronic Care Professional (CCP) are a plus.
• Willingness to obtain CCM certification within 18 months of employment is favorable.
