Bilingual Care Team Manager, Spanish Speaking

📋 Description

• In the role of Care Team Manager, you will be integral to our holistic, interdisciplinary care approach.

• Your responsibilities will include outreach and engagement with individuals facing Serious Mental Illness, Substance Use Disorder, homelessness, or those with significant medical needs.

• You will prioritize relationship-building and offer continuous support to enhance the quality of life for individuals eligible for the Enhanced Care Management benefit.

• You will oversee six interdisciplinary pods, providing support to up to 18 Lead Care Managers (LCMs) simultaneously to ensure high-quality patient care and effective engagement strategies for those with complex medical and social challenges.

⛳️ Requirements

• A minimum of 2 years of experience managing large teams of individual contributors, preferably within a healthcare or community care environment.

• More than 2 years of experience in community engagement, patient navigation, or social work.

• Background in care teams, adept at navigating multidisciplinary collaborative care models.

• Bilingual in English and Spanish.

• Strong cultural fluency and a comprehensive knowledge of community resources.

• Proven experience working with individuals facing complex chronic challenges, such as homelessness or severe mental illness/substance use disorder.

• Technical skills in CRM databases, basic Excel, Word, email, and video conferencing platforms.

• A quiet, HIPAA-compliant workspace at home with dependable internet and cell phone access is required.
